These are great pictures.

What kind of light do you use one each side? How many watts per bulb?

I want to photograph my new grand daughter in the house, so maybe I need to use lights on each side.

When you have the two lights on, do you leave the camera flash off?

Lots of questions. =P
First of all the type of lights I use are Alien Bees B400s (150W). They are flash studio lighting so you need a SLR to plug them into or you can use a wireless set up putting the transmitter where you would normally attach a flash. No I do not use my flash that attaches to my camera (Never use built in flash) when I have studio lights on. I also use soft boxes on the studio lights most of the time.

Though for what you want to do you can just buy a flash that attaches to your camera & a diffuser and bounce the flash off of the celling and it still gives a nice result.
